Tick the Key Result Areas from the hospital's performance contract that you worked on. Most nurses pick 2 to 6. Search by a word (e.g. "eye", "theatre", "documentation") or by your unit's code. If yours is not listed, add it at the bottom.

Monthly results

For each month, choose two of your tasks (a third is optional) and enter each one's Output Status: your score for that task in that month. Some tasks are percentages (0 to 100); others are numbers, such as how many screenings or trainings you held. Under each box you'll see your contract target and the rating scale. Falling short of the target is not wrong: enter your true figure and your supervisor rates it on that scale. Leave a month blank if you do not have its figures yet.
